    jQuery Dynamic Form description

    This jQuery plugin adds dynamic form fields to contact forms

    Fields are added at the user's request, based on a predetermined form template built by the admin.

    jQuery Dynamic Form adopts different behaviours if the duplicate buttons ( plus & minus ) are inside the template or outside.

    The plugin can add one or more fields at the same time, also combined with sleek animations.

    Requirements:
